Case Management

Cultural Art Center
Everyone has something to share; their ideas, their joy, or their pain. Art can provide individuals with intellectual and/or developmental disabilities with a means to share. On a higher level, talented artists are able to earn money by selling their work in our online store.
Residential Supports
Services provided uniquely to the needs of each individual in a community home environment. Individuals are placed in homes of their choosing in safe, clean neighborhoods. In this program, 24-hour direct support staff ensures all individuals are safe, happy, and able to enjoy all the freedoms of an independent lifestyle. This program supports Level I and Level II individuals.
Community Living Support
Training or assistance provided individuals to promote independence to an individual not receiving residential support. This can include participating in community activities, household care, and shopping.
Respite
The love, service, and support provided by family caregivers can be physically and emotionally draining. To help these loved ones relax and recharge, Evergreen provides a variety of respite care services.
Supported Employment
This program ensures all individuals who want to work have the opportunity. We strive to assist people with disabilities to obtain real work for competitive wages. Evergreen Life Service staff are trained as effective job coaches and provide assistance that facilitates the individual to thrive and be a productive contributor to the work force.
Adult Day Training
Services that provide a positive environment for individuals to learn activities of daily living, promote positive interaction with others, and build relationships. These supports can be either onsite, in a community and nonresidential setting, or offsite services.
Behaviors Support
This program gives the individuals we serve the ability to participate in daily meaningful skills and to develop adaptive and social skills routinely utilized in the community. Behavior supports provide techniques and methods to influence or change behavior(s) that are unhealthy.
Personal Assistant
Enables a person to accomplish daily tasks, including hands-on assistance; reminding, observing, guiding or training during an activity; assistance in managing medical care and transportation to access community services, activities and appointments, if these are not available under the Medicaid Program. Personal Assistance Services are available only to those who live in their own residence or in their family’s residence.
